Who We Are

Home Health Representatives

 Home Health Care is a program designed to assist home bound patients who are recuperating from an illness, disability, surgery or injury that require extra supervision at home to guide them into optimal healing.

Advanced Nurses

 Available for high level IV therapies, TPN feedings, Colostomy Care, pleurex drains, and advanced wound care. 

Skilled Nursing

 RNs & LVNs provide patients with medication management education, IV therapies, wound care, intramuscular injections, disease process education, and catheter care. In addition, they monitor the patient to ensure medications are working effectively while communicating with the primary care physician to ensure optimal healing.

Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from mobility.  Our Physical Therapists can assist you with tasks such as mobility and exercising. Assessment of the social and emotional factors related to the beneficiary's illness, the need for care, their response to treatment, and adjustment to care. Assessment of the relationship of the medical and nursing requirements to the home situation, financial resources, and the community resources available. Services provided on a short-term basis (two to three visits) to a beneficiary's family member or caregiver when it is shown that a brief intervention is necessary to remove a clear and direct impediment to the effective treatment of the beneficiary's medical condition or to the rate of recovery. Appropriate action to obtain available community resources to assist in resolving the beneficiary's problem.

Overall, practitioners offer strategies for their patients to manage daily activities while at the same time cutting back on the risk of injury or further decline. This is often a give and take process that will ultimately determine the correct fit between the abilities of the patient, the home environment, and the limitations of the illness or injury. Their main goal is to improve efficiency and maximize positive outcomes for patients.

 SLPs assess speech, language, cognitive-communication, and oral/feeding/swallowing skills to identify types of communication problems (articulation; fluency; voice; receptive and expressive language disorders, etc.) and the best way to treat them.
